Eczema Isn't Just a Skin Problem: What Ayurveda Sees That Modern Treatments Often Miss

If you suffer from ‘Eczema’, it seems like you will never be free of it because of the persistent itching, dry and inflamed patches, and returning redness no matter what creams or medications you use to treat it. You have likely been in this cycle many times: flare-up, temporary relief provided by treatment, and then the rash comes back. Many times the focus has been almost exclusively on what can be seen, the skin, but what if your eczema is not just a skin condition?

Recent emerging research on the gut-skin axis supports that there is influence from digestive health on certain skin conditions like eczema. Ayurveda has maintained this concept for centuries, with many chronic skin conditions being reflections of an internal imbalance found within the body, which can often be caused by digestive issues.

In other words, your eczema may be a symptom on the outside, which is your skin, while the cause of that symptom originates from your digestive system on the inside.

Understanding Eczema Beyond the Surface

Atopic dermatitis, which is also known as 'eczema', is a long-lasting skin condition that is caused by scratching, redness, dryness, irritation and repeated outbreaks (flare-ups) of eczema. There are many reasons that may contribute to eczema, including genes, allergies, environmental triggers, and problems with the immune system; however, the reason that some people have bad eczema even when they are treated through conventional methods is not known.

Many eczema sufferers have noticed that their eczema can become worse if they consume certain food items or if they experience digestive pain. This information shows that something other than skin is affecting the process that causes eczema. The association between the gut and skin has many researchers and healthcare providers looking into how good digestive health can affect chronic inflammatory skin disorders like eczema.

The Gut-Skin Axis: A Modern Perspective

There are trillions of microorganisms that live in a person's digestive system and are called the gut microbiome. The gut microbiome does much more than just help a person to digest food; it is essential for regulating the immune system, absorbing essential nutrients, regulating inflammation, and protecting the body's natural defence systems from infections.

If a person has an imbalance in their gut microbiome, their immune system may become overactive and cause chronic inflammation throughout their body.

There is evidence to suggest that poor gut health may be linked to a variety of skin diseases, including, but not limited to, eczema. This connection between gut and skin is known as the gut-skin axis. When a person's gastrointestinal system is affected by certain conditions, the production of inflammatory signals may be increased, which can contribute to skin discomfort and flaring of the skin condition. Although eczema is often seen on the surface of the skin, it is sometimes caused by deeper issues in the body's internal systems.

What Ayurveda Says About Eczema and Digestion

Healthy digestion is believed to be the basis of healthy skin according to Ayurvedic principles. Ayurveda places great importance on the concept of Agni (digestive and metabolic "fire"), which is the energy required to convert food into nutrition. When Agni functions properly, nutrients are assimilated correctly, and the tissues remain healthy. When digestion is impaired or weak due to a poor diet, stress, eating out of sequence, lack of sleep, and excessive amounts of processed food, the digestive system will not be able to perform its job correctly.

Incomplete digestion can lead to the creation of Ama, which is the Ayurvedic term for toxic metabolic waste that results from improperly digested food. It is believed that Ama can circulate throughout the body, cause blockages in the normal physiological pathways, disturb the functional balance of each of the three Doshas and contribute to many health issues, including chronic skin problems like Vicharichika/Eczema; there is believed to be a correlation between both ‘Ama’ (toxins) and the progression of these types of skin disorders.

Why Treating Only the Rash May Not Be Enough

Traditional treatments concentrate on efforts that will provide symptomatic relief by lowering inflammation, alleviating itch, and controlling flare-up events. Although these interventions can improve an individual's quality of life tremendously, if there is underlying digestive dysfunction that has not been appropriately addressed, symptoms can continue to recur. Thus, many people have experienced periods of temporary improvement followed by multiple relapses.

Ayurveda takes a root-cause approach; rather than focusing simply on how to suppress the rash, it also looks at why the rash occurred in the first place. To find the underlying cause of the rash, Ayurveda evaluates the digestive system’s strength, dietary habits, lifestyle habits, stress levels, and metabolic equilibrium in order to identify the internal environment which may be contributing to the rash.

The Ayurvedic Approach to Managing Eczema: Treating the Root Cause, Not Just the Rash

Ayurveda views eczema (Vicharchika) as more than a localized skin disorder. It is considered a manifestation of internal imbalances involving the ‘Doshas’, particularly Kapha and Pitta, along with the accumulation of ‘Ama’ (metabolic toxins) and vitiation of ‘Rakta Dhatu’ (blood tissue). Therefore, the Ayurvedic approach focuses on correcting the underlying pathology rather than merely suppressing visible skin symptoms.

The first step in management is the restoration of Agni (digestive fire). Since impaired digestion is believed to be the foundation of Ama formation, improving digestive strength becomes essential. Dietary modifications, individualized herbal formulations, and lifestyle corrections are prescribed to support digestion and reduce the production of inflammatory metabolic waste. However, in chronic, recurrent, or severe cases of eczema, Ayurveda often recommends Shodhana Chikitsa (purificatory therapies) through Panchakarma to eliminate deeply seated Dosha imbalances and toxins from the body.

Vamana (Therapeutic Vomiting)

Vamana is the most important Panchakarma treatment for treating conditions that have a lot to do with an excess of Kapha Dosha. This will benefit those with eczema who often have a history of skin lesions that ooze, itch a lot, are thick or repeatedly become red and inflamed because of Kapha-related issues. Therefore, Vamana can help treat the symptoms of eczema in some patients if done after preparing the patient properly.

Vamana is carried out in a medically supervised environment to manage the vomit as it clears out extra Kapha, as well as any toxins, by inducing a vomiting reaction from the upper gastrointestinal (GI) system. After inducing vomiting and clearing out extra Kapha, many patients will experience systemic diminishment of their body's inflammatory response and improvement of their metabolic function, and thus lower the rates of flaring back up again on the skin. Therefore, Vamana is a helpful therapy for breaking the chronic cycle of eczema flare-ups.

Virechana (Therapeutic Purgation)

Virechana is a very effective therapy for treating several skin disorders through the use of therapeutic purging. One of the main causes of eczema is an excess of Pitta Dosha in the body (burning-red-cold), as it presents with burning sensations, redness in the skin, an inflammatory response from the body, and general skin irritability, all induced by the excess Pitta. Virechana helps reduce the excess Pitta from the body and the toxins created from the excess through the use of regulated therapeutic purging methods.

Raktamokshana (Bloodletting Therapy)

Raktamokshana is the act of removing impure blood from the body in an effort to cleanse it of impurities, thereby aiding the healing process of many skin disorders. Raktamokshana can be accomplished by removing the blood using different methods, but all are based on the same principle – the removal of vitiated (and therefore, infected or diseased) blood removes the inflammatory agents that contribute to imbalance and creates an opportunity for the body to heal itself.

In addition, the treatment also improves the circulation of the blood vessels in the area where the blood is drawn. One way to accomplish this is through the use of leeches (Jalaukavacharana). Historically, leeches have been used in cases of acute inflammation of the skin, with symptoms consisting of redness, heat, swelling and/or taste. By removing vitiated blood from a localized area and enhancing blood circulation, Raktamokshana can support the body’s ability to heal and contribute to the reduction of inflammation. Due to the efficacy of Raktamokshana, it continues to be one of the most recognized Ayurvedic treatments for chronic inflammatory skin disorders.

Shirodhara (Therapeutic Oil Streaming)

Research shows that one of the most significant factors in contributing to an increase in eczema flare-ups is stress. Many individuals diagnosed with eczema experience symptoms of eczema during stressful times, such as having anxiety or not getting a good night's rest due to stress, and displaying emotional difficulties.

According to Ayurvedic medicine, there is a strong relationship between the mind and skin. As a result, many Ayurvedic therapies are developed to help calm or soothe the nervous system.

Shirodhara is a therapy that uses a steady stream of medicated oil, buttermilk, or other appropriate medicinal liquids to be poured continuously on the forehead in a rhythmic fashion. Shirodhara is extremely relaxing for the mind, and it reduces levels of stress hormones within the brain, improves the quality of sleep, and promotes tranquillity of the mind. Therefore, while Shirodhara may not directly eliminate the presence of an eczema lesion, it may help with the stress levels of the patient, thus making it a useful adjunctive treatment for those who are living with and managing chronic skin diseases.

Lepam (External Herbal Applications)

The use of herbal pastes made especially for Lepam applications on the skin is an example of utilizing herbs to heal skin conditions such as eczema. Lepams are made up of a variety of medicinal herbs that possess anti-inflammatory, soothing, antimicrobial and healing properties; this allows for the selection of these herbs based on each patient’s Dosha predominance and stage of disease.

When utilising Lepam to treat eczema patients, there is a significant reduction in the redness, itching, burning sensations, oozing and discomfort associated with this condition. In contrast, most commercial topical products aim primarily at treating the symptoms they produce, while Lepam supports the natural healing process of the skin in conjunction with the internal therapeutic approach. Combining these two approaches generally provides a more complete treatment regimen than either approach alone.

“Look Beyond the Rash” Because healthier skin often begins with a healthier gut
